The Fungus Testing
Laboratory has been inspected and assigned a
registration number by the CDC to handle Coccidioides
immitis and Coccidioides posadasii. Handling
of select agents is governed by federal regulations
outlined in 42 CFR € 73 Possession, Use, and Transfer
of Select Agents and Toxins; Interim Final Rule. By
assigning this registration number, the CDC has
confirmed that the FTL meets the requirements specified
in 43 CFR, Part 73, and is equipped and capable of
safely handling the agents listed above.
Submission of Select Agents for Diagnostic Testing:

Contact the FTL @ 210-567-0406 to
request EA101 signed and dated by the FTL Recipient,
PI, and RO. This will be sent to you via FAX

Upon receipt of EA101 with completed
Section 1 (Recipient Information) & 2 (Select
Agent Description), you must fill in information
requested in section 3 (Transferor Information),
section 4 (Shipping Information -
do not
complete the Date agent received box), and List
of Select Biological Agents and Toxins Shipped
section.

Ensure your RO signs the block under
the List of Select Biological Agents and Toxins
Shipped section.

FAX completed document to CDC @

The CDC will assign this document a
CEA number. Write this CEA number above Section 4
on the completed EA101.

Submit the completed EA101 including
the CEA number and your RO Signature with the
specimen. Also submit a completed FTL Requisition
for testing with the specimen.

The FTL will complete the Date Agent
Received block in section 4 and send notification of
receipt to you and to the CDC.
